Multaq (Dronedarone) - Summary

WARNING: HEART FAILURE

MULTAQ is contraindicated in patients with NYHA Class IV heart failure, or NYHA Class II — III heart failure with a recent decompensation requiring hospitalization or referral to a specialized heart failure clinic [see Contraindications (4) ].

In a placebo-controlled study in patients with severe heart failure requiring recent hospitalization or referral to a specialized heart failure clinic for worsening symptoms (the ANDROMEDA Study), patients given dronedarone had a greater than two-fold increase in mortality. Such patients should not be given dronedarone [see Clinical Studies ].

MULTAQ SUMMARY

Dronedarone HCl is a benzofuran derivative.

MULTAQ® is indicated to reduce the risk of cardiovascular hospitalization in patients with paroxysmal or persistent atrial fibrillation (AF) or atrial flutter (AFL), with a recent episode of AF/AFL and associated cardiovascular risk factors (i.e., age greaer than 70, hypertension, diabetes, prior cerebrovascular accident, left atrial diameter greater than or equal to 50 mm or left ventricular ejection fraction [LVEF] less than 40%), who are in sinus rhythm or who will be cardioverted [see Clinical Studies ].

Rhythm- and rate-controlling effects of dronedarone in patients with atrial fibrillation (from the ATHENA trial). [2011.04.01]

Effect of dronedarone on exercise capacity and cardiac function in patients with severe left ventricular dysfunction and compensated stable heart failure. [2010.12]
PURPOSE: Dronedarone is a novel multichannel blocker with antiadrenergic and vasodilatory properties. The aim of this study was to investigate the effects of dronedarone on functional capacity in patients with severe left ventricular (LV) dysfunction and compensated stable heart failure (HF)... CONCLUSIONS: Short-term treatment with dronedarone did not affect exercise capacity and did not decrease LVEF in patients with severe LV dysfunction and compensated HF.

Clinical Trials Related to Multaq (Dronedarone)

A Study to Evaluate the Effect of Ranolazine and Dronedarone When Given Alone and in Combination in Patients With Paroxysmal Atrial Fibrillation (HARMONY) [Recruiting]
The purpose of this study is to evaluate whether treatment with ranolazine or low dose dronedarone reduces atrial fibrillation burden (AFB) in subjects with paroxysmal atrial fibrillation (PAF), and whether combination therapy (ranolazine and low dose dronedarone) is superior to individual drug therapy in reducing AFB.

Dronedarone in Pacemakers Patients With Paroxysmal Atrial Fibrillation [Not yet recruiting]
The purpose of this study is to accurately investigate the efficacy of dronedarone in maintaining sinus rhythm and decreasing AFB in patients with paroxysmal atrial fibrillation as compared with placebo. This has never previously been performed using pacemaker Holter monitoring which provides detailed information of atrial arrhythmia patterns the entire study period. Additionally detailed patient symptom self assessment and questionnaires will be collected. The study design will be double blinded crossover with each phase lasting 3 months.

Effects of Dronedarone on Cardiac Geometry and Function in Patients With Atrial Fibrillation and Left Atrial Enlargement [Not yet recruiting]
Primary Objective:

- Evaluate the effect of dronedarone versus placebo (standard therapy) in slowing the

progression of adverse left atrial remodeling in patients with atrial fibrillation (AF) following 12 months of treatment

Secondary Objectives:

- Evaluate the effects of dronedarone versus placebo on left atrial function

- Evaluate the effects of dronedarone versus placebo on left atrial dimension

- Evaluate the effects of dronedarone versus placebo on left ventricular function (LVEF,

E, E', A, E/E')

- Evaluate the safety and tolerability of dronedarone

Dose Ranging Study of Dronedarone for the Control of Ventricular Rate in Japanese Patients With Permanent Atrial Fibrillation [Recruiting]
Primary Objective:

- To assess the efficacy of dronedarone versus placebo for the control of ventricular rate

in patients with permanent atrial fibrillation (AF).

Secondary Objective:

- To assess the safety and tolerability of dronedarone after repeated oral doses of 300

mg, 400 mg, or 600 mg twice daily in the selected population.

- To document SR33589 and SR35021 trough plasma levels at steady state.

Dronaderone to Prevent Recurrent Implantable Cardioverter Defibrillator (ICD) Shocks [Not yet recruiting]
Recurrent ICD shocks are a serious problem in pts with ICD. Treatment failure with other antiarrhythmics and ablation are common. Dronedarone is a new antiarrhythmic drug with ion channel properties similar to amiodarone.

Several case reports have shown promising results with Dronedarone for this patient population.
