Make the Diagnosis: A Four-Year-Old Boy with Right Shoulder Pain

(MedPage Today) -- Case Study: Six months prior to this presentation, a four-year-old boy was brought to a hospital with a one-week history of right shoulder pain. Radiographs revealed a mildly expansile lytic lesion with cortical thinning and intratumoral calcification in the proximal part of the right humerus. CT demonstrated multiple additional lesions in the right scapula and radius. A sample obtained from the humeral lesion by needle biopsy was diagnosed as enchondroma. On physical examination six months after the biopsy, an osseous hard mass was palpable over the proximal part of the right humerus, with local tenderness. Radiographs, shown here, revealed tumor growth (a) and endosteal lysis in the right scapula (b) and radius (c). MRI demonstrated a lobulated humeral lesion with no extraosseous extension. Pain, the increased size of the lesion relative to radiographs from six months prior, and the lytic area with cortical destruction prompted surgeons to perform an open biopsy. Pathologic examination showed multinodular growth of hyaline cartilage, atypical chondrocytes, and several areas where the tumor closely intermixed with lamellar bone. What is the diagnosis?
